Strengthening Care for Patients with Complex Needs

In 2025, Marin Community Clinics expanded support for patients with complex health and social needs through PATH-CITED Round 3 funding. This investment strengthened our Enhanced Care Management program by supporting staff training and workspace improvements.

Six members of our Complex Care team completed specialized training to better support patients facing challenges such as chronic health conditions, housing instability, and barriers to care. This training strengthened skills in care coordination, patient engagement, and team-based support.

At the same time, four dedicated workstations were created for the Enhanced Care Management team. These spaces improve communication, collaboration, and efficiency, allowing staff to respond more quickly to patient needs.

Together, these investments help Marin Community Clinics provide more coordinated, timely, and compassionate care for patients and families who need additional support.

Building Community Preparedness Initiative

In 2025, Marin Community Clinics participated in the Marin Community Foundation’s Building Community Preparedness Initiative, a countywide effort to strengthen community health and readiness. Through meetings and workshops, MCC collaborated with community organizations, public agencies, and healthcare partners to better understand how weather and environmental conditions affect patient health and access to care.

This collaboration strengthened partnerships and expanded MCC’s role in supporting families during extreme weather and other community challenges. By connecting emergency preparedness with patient care, MCC is better positioned to support patients before, during, and after emergencies, ensuring continuity of care when it matters most.

Participation in this initiative also helped identify opportunities for future collaboration, improve patient education, and strengthen facility planning. Together, these efforts support Marin Community Clinics’ ability to provide consistent, high-quality care and prepare for future community needs across Marin County.
