2025 Annual Report

One Roof. One Team.
Thousands of Families Served.

How our Whole Family Care model delivered deeper, more connected care in 2025.

For years, great healthcare meant treating the person in front of you. At Marin Community Clinics, we still believe that — but in 2025, we took it further.
The shift from Whole Person Care to Whole Family Care was not a new program or building. It was a change in how we approach care. Instead of asking, “What brings you in today?” our teams began asking, “Who else in your family can we help?” This shift created a care model focused on supporting the health of the entire family.

The impact is already clear. In 2025, we served 38,221 people through 236,730 visits, offering primary care, dental, behavioral health, and wellness services as one connected experience. When a parent visits, we consider their child’s needs. When a grandparent receives care, we think about the family supporting them. Care extends beyond the individual to the household.

Whole Family Care strengthens prevention and long-term health. By supporting families together, we help build healthier routines, address challenges early, and create continuity across generations.

“By treating the whole family, we strengthen our community.”
— Ana Tafolla, Family Support Specialist
