Marin Community Clinics 2015 Annual Report: Closing the Gap

Every year is an exciting one at Marin Community Clinics, but 2015 was particularly eventful. The dramatic increase in the number of patients continued, we added new services and staff, and we enhanced our financial health. Most importantly, we made great strides closing the gap – people who had never received medical or dental care are now coming to our clinics. We are full!

2015 at a Glance*

■ 9 clinics at 6 sites (medical, dental and teen) – the third largest health care provider in Marin
■ 35,000 patients and 161,128 patient visits
■ Patient–centered Medical Home model
(Level III –highest possible designation)
■ 310 staff
■ 7 active clinical volunteers
■ Patient Call Center averaged 1,000 calls/day
■ Specialty Referral Team received 1,500 referral requests/month
■ $35 million annual budget
■ $1.7M raised through philanthropy

Community
■ Weekly Health Hubs in San Rafael and Novato served an average 450 individuals/week with free, nutritious food, health education and screenings, and exercise. From June – December, an estimated 38,000 household members benefited.
■ Counseled or enrolled 3,681 community members in health insurance programs

Patient Growth

We continue to be the largest safety net provider of primary care in Marin, serving 35,000 patients – a significant portion of Marin’s population. Over 50% of our patients are children.

Marin Community Clinics takes great pride in the high quality of care we provide to our patients.
Patient Satisfaction
■ 93% of our patients would recommend us to family and friends
Prevention
Our focus on prevention and mental health management has resulted in:
■ 26% increase in controlling high blood pressure among hypertensive patients
■ 26% improvement in controlling blood sugar in diabetic patients
■ 45% improvement in follow up care for patients newly diagnosed with depression
Awards and Recognitions
■ Maintained high ratings in our most recent federal survey
■ Received $60,780 in awards from the federal Health Resource Services Administration for quality leadership
■ Received $1.3M incentive award from Partnership Health Plan for quality efforts, including prevention and diabetes management
■ Received 7 grants totaling $555,420 for quality advancement
■ 33 additional clinicians designated by the federal government as skilled in using the Electronic Health Record

What’s Coming Up

Our theme for 2016: “Quality Health Care – for All”
Just as the healthcare landscape is changing, so is Marin Community Clinics. Although our core mission will never change, we now have a broader view of who we serve.

In 2016, look for the following:
■ Opening of our expanded Novato Clinic: $4 million in renovations – 21 medical exam rooms, 3 additional dental chairs (for a total of 9), and a new radiology service
■ New clinics focused on serving older adults
