MCC Honored with CalAIM Award for Youth Services

In June, Marin Community Clinics (MCC) was proud to receive the CalAIM Make a Difference Award from Partnership HealthPlan of California. This honor recognizes MCC’s leadership in providing Enhanced Care Management (ECM) for youth—along with a $10,000 grant to help us expand this critical work.

Whole-Person Care for Youth in Need

ECM is more than medical treatment. It is high-touch, wraparound care designed for young people facing life’s toughest challenges—chronic illness, unstable housing, food insecurity, or family trauma. Our care teams connect youth not only to doctors and mental health providers, but also to housing resources, school support, and community services. This approach helps build stability and restores hope for the future.

A Story of Transformation

One young patient—a 16-year-old living with type 2 diabetes, hypertension, and depression—was struggling to manage his health and keep up in school. Through ECM, he received counseling, care coordination, and family support. With MCC’s help, his family secured vital housing and financial resources. Today, he is back in school, healthier, and more optimistic about his future.

“This is what whole-person care looks like,” says Cayenne Bierman, Director of Complex Care. “We meet patients where they are and help them move forward with the tools and support they need.”

Expanding Impact Together

Since extending ECM services to youth in 2023, MCC has supported more than 265 young people across Marin County. These are teens who might otherwise fall through the cracks—now receiving consistent care, stability, and encouragement to thrive.

But the need is growing. With your partnership, we can sustain and grow this life-changing program, ensuring Marin’s most vulnerable youth have the support they need to succeed—today and for years to come.
