As a County we have many resources that can help people but they too often work in isolation from one another. This can result in duplication of services or gaps in care. CalAIM (formerly “Whole Person Care”) solves these problems by providing support for the individual in the form of one-on-one mentoring and, additionally, coordination of care among service organizations. Marin Community Clinics is a part of this system in Marin County, offering Behavioral Health Case Management and Medical Case Management.
