Referral Coordinator
The Referral Coordinator, under the immediate supervision of the
Referral Director, provides advocacy, referral, and follow-up to
patients in conjunction with the medical plan of care. Completes
eight to ten follow-up’s a day. Completes ten to twelve referrals a day.
Responds to or makes 15-25 calls a day. Seeks guidance from Referral
Director, Nurses and Providers as needed for clinical resources.
This position provides extensive support to providers and patients.
Maintains relationships with specialists, with outside partners in
health care, ie. schools, county
programs, other providers, hospitals to coordinate services and
support the community.
Responsibilities:
- Independently review patient medical records by reading provider notes to ascertain what intervention is needed.
- Follow up in a timely manner on pending lab reports, x-rays, appointments, or other planned evaluation and treatment as needed to coordinate referrals.
- Make arrangements for diagnostic or screening tests as requested by specialist or PCP.
- Work with patient to promote follow through on appointments and provider recommendations.
- Track and follows up on status of all pending authorizations, appointments, applications, insurance eligibility and referrals.
- Manage multiple tasks and work on special projects as assigned by the Referral Director.
- Coordinate work for staff members who are assigned to assist in processing referrals.
- Process referrals and follow-up, arrange services with specialists in a timely manner.
- Inform provider if there is an unusual delay.
- Utilize MS Outlook software to set reminders and course of follow up referrals.
- Maintain complete, current, and accurate documentation which includes date, action taken, contact information of referral, current patient demographics and insurance eligibility in conjunction with medical plan of care.
- Attend RC and All Staff meetings as scheduled by Referrals Director.
- Handle patient complaints in professional manner and refer patients as necessary to appropriate clinic management if patient’s complaint is too complex for RC to handle.
- Respond to urgent patient or staff inquiries and requests within a 24 hour period.
- Respond to patient, staff or outside service calls within a 3 hour period.
- Complete applications for Babcock Endowment Foundation, Operation Access, and other community programs.
- Request prior authorizations from: HFHN, HFBC, Cal-Kids, CMSP & Medi-Cal insurance companies for medications, specialist office visits/follow-ups, medical equipment and procedures.
- IAssist patients by explaining process of referrals, services or resources. Display excellent customer service when doing so.
- Fax pertinent medical records, demographic information, insurance eligibility to different specialist offices.
- Maintain relationships with outside services by working in a professional manner, following specialists’ protocols, and providing patient information in a timely manner and displaying a professional attitude.
- Manage schedules for In-House specialists.
- Maintain waiting lists and appointment scheduling of In-House specialty clinics, or notify person in charge of specialty care of patients who need appointment. Work with specialist offices to confirm clinic schedule.
- Confirm In-House specialist appointments with patient prior to their appointment via mail and on the telephone.
- Handle all case management follow-up issues/requests after the patient has been seen by the specialist.
- Maintain and coordinate other assigned specialty referrals, such as Cardiology Associates, Kaiser as assigned.
Requirements:
- High school diploma/equivalent required; two years or more post-secondary school education preferred
- Strong project coordination skills preferred
- Experience using various software programs including Microsoft Outlook, Word, Access, Excel
- Medical assistant or other support staff role in a medical setting with at least 2 years experience
- Ability to read and interpret medical records. Excellent written and verbal communication skills. Ability to chart actions clearly. Ability to work with patients and representatives of other organizations on the phone
- Bi-lingual English-Spanish or English-Vietnamese. Must be sensitive to multi-cultural, multi-ethnic issues. Knowledge of medical terminology preferred
Call Center Operator/Scheduler
This position is the entry point for most Marin Community Clinics patients. As such, a high level of customer service must be practiced. Members of the Call Center team must be calm, helpful and supportive while assisting patients and other callers. Members of the call center team will assist patients in their request for services, schedule appointments, update demographic information, and otherwise ensure a satisfactory and productive visit to MCC. The position is full time, located in San Rafael.
Responsibilities:
- Professionally represent Marin Community Clinics through positive, courteous and professional patient contact.
- Adhere to the strict HIPAA law regarding health information protection.
- Effectively and efficiently answer, screen and route calls utilizing a multi-line phone system.
- Make patient appointments utilizing the practice management system data base appointment scheduler.
- Maintain integrity of patient data by verifying and correcting information within practice management system.
- Ensure appropriate patient scheduling according to Call Center Appointment Rules.
- Ensure customer satisfaction by analyzing complaints and proactively problem solving.
- Conduct daily patient appointment confirmation phone calls as necessary.
- Correctly respond to inquiries about the organization and provide callers with address, directions and other information.
- Route all incoming mail and prepares outgoing mail.
- Work with team, maintaining consistent attendance, punctuality, and superb customer service.
- Perform general clerical duties, including filing, typing, maintaining records, and other misc. duties.
Requirements:
- High School diploma with excellent, professional interpersonal and telephone skills in both English and Spanish; BILINGUAL ENGLISH/SPANISH IS REQUIRED
- Prior call center experience is preferred and/or prior experience in a medical office setting
- Must have demonstrated experience serving under-privileged clients in a professional and courteous manner
- Excellent organizational skills and attention to detail
- Demonstrated ability to competently work in a team environment and support co-workers Requires high level of initiative and the ability to follow-through on assignments independently to completion

Licensed Clinical Social Worker (LCSW)
Marin Community Clinics is seeking a bilingual (English/Spanish) LCSW (Full or Part time) with an emphasis on outpatient, Pain Management care and practice.
Responsibilities:
- Provide mental health services to clients at one or all 3 of our primary healthcare clinics located in Greenbrae, San Rafael and Novato. Services include counseling, crisis intervention, assessment, service planning, and follow-up.
Requirements:
- Bilingual English/Spanish required
- Must be a Licensed Clinical Social Worker within the State of California
- Experience providing clinical counseling services to individuals with multiple challenges including pain management, mental illness, addiction, HIV/AIDS, and other health and social issues
- Demonstrated ability to effectively and sensitively provide care to people from different cultural groups
- Ability and experience working as a member of a multi-disciplinary service team

CPSP Case Manager - Supervisor
(Comprehensive Perinatal Services Program)
Reporting to the Women’s Health Director and Clinic Director, this position entails Client Orientation to Comprehensive Perinatal Services Program,
initial assessment, trimester reassessments, post-partum assessments, interventions, and case coordination for pregnant women.
Specific components of CPSP Program include: nutrition, health education,
links to WIC Program, mental health services, family planning, and
pediatric care.
This position will further supervise the activities of other CPSP case
managers, act as a lead resource for CPSP program, and directly
participate in collaborative Obstetrical Team.
Responsibilities:
- Conduct face to face interviews with all prenatal patients
- Identify opportunities to expand service and increase patient access to CPSP program
- Assure all client charts are documented whenever telephone and face-to-face contact is made
- Provide ongoing orientation, assessments, and classes. Make appropriate referrals when needed
- Conduct pregnancy testing and counseling and family planning counseling
- Ensure all patient appointments for care establishment and return visits are made
- Case manage prenatal program, including reviewing chart at each visit, as well as periodic audits of all prenatal patient charts to ensure quality of care
- Attend recommended job related trainings provided by CPSP program
- Assist Women’s Health Director in data collection and reporting
- Assist with staff evaluations
- Obtain full knowledge of MCC Emergency Protocols
Requirements:
- Must be at least 18 years old with High School Diploma

- Minimum 1-year paid Perinatal (OB-GYN) experience
- Bachelor’s Degree preferred in community health, health services, or relevant community experience
- At least 1 year Experience with CPSP required
- Ability to work independently, identify, and prioritize tasks

Psychiatrist
Marin Community Clinics is seeking a Psychiatrist (Part time) to provide quality care to patients of MCC, consistent with her/his training and expertise, and appropriate to the ambulatory care setting provided. Responsibilities:
- Evaluate, diagnose, and treat mental, emotional, or behavioral disorders affecting children, adolescents, adults and their families.
- Diagnose disorders, including nature and extent, determine the appropriate course of treatment, and discuss these treatment plans with the patient and/or patient guardian.
- Prescribe medication when necessary.
- Act as an advocate for the best interests of the patient.
- Reports to the Chief Medical Officer of Marin Community Clinics.
Requirements:
- Familiarity with standard concepts, practices, and procedures in pediatric and/or adult psychiatry.
- Valid and unrestricted license issued by the Medical Board of California to practice psychiatry.
- 2-4 years of psychiatry experience.
- A current registration issued by the DEA allowing the prescription of drugs.
- A current BLS certification.
- Willingness and ability to follow clinic policies, objectives and mission established by regulations, grants, management or the Board of Directors.
- Proficiency in medical Spanish is desired.
Site Medical Director
Due to our recent growth and expansion, we seek an on site Medical Director for our San Rafael clinic location. Reporting to the Chief Medical Officer, the Site Medical Director provides guidance, direction and clinical staff oversight to the MCC San Rafael Clinic’s medical staff concerning all aspects of primary care. Reviews and maintains medical standards which assure the quality of medical services provided to all patients treated at the clinic.
Oversight Responsibilities:
- Participate in provider meetings, QA meetings, Center Leadership Team meetings and other professional activities as requested by the Chief Medical Officer.
- Supervise and support licensed clinicians through goal setting, coaching, and mentoring.
- Provide support for quality initiatives.
- Participate and/or lead medical center projects that improve access and quality of care.
- Assist CMO with interviews, hires, and orientation of licensed clinical providers.
- Objectively evaluate and document performance of providers and escalate to CMO when need for corrective action is deemed appropriate.
- Lead/participate, as appropriate, in collaborative team approaches to developing and enhancing an integrated system of care.
- Assist in the development and implementation of medical office or administrative policies related to the delivery of care.
Clinical Responsibilities:
- Provide for the diagnosis, treatment and follow up of health center patients within the scope of his/her license to practice medicine.
- Provide back-up for mid-level providers on site, or by phone during the day or after hours.
Requirements:
- Graduate of an accredited medical school.
- Board certified MD or DO with current California License and DEA certificate.
- Three or more years experience providing primary care in a family practice setting preferred.
- Demonstrated clinical skills.
- Computer skills required.
- Current BLS certification.

Medical Assistant
The Resource Medical Assistant is cross trained in all functions of the clinic including, but not limited to, front office, appointment desk, phones, medical records and case management. The candidate must possess an interest in all aspects of clinic operations and be able to function in multiple roles and be able to multi-task.
Requirements:
- Equivalent MA curriculum from two-year college or technical school; or six months to one year related experience and training; or equivalent combination of education and experience.
- Medical assistant certification preferred.
- Current CPR.

Family Practice Urgent Care Physician
Marin Community Clinics is seeking a Family Practice Urgent Care Physician, full time. The physician provides quality urgent care to patients of MCC, consistent with her/his training and expertise, and appropriate to the ambulatory care setting provided. Ideal candidate would be a residency trained, board certified, physician with a track record of being able to work in a busy urgent care practice or clinic.
